    Application

    • 1). Contact your country recorder's office for particulars for your jurisdiction. Many counties have websites that also offer information on birth certificates.

    • 2). Obtain an application for a certified copy of a birth certificate. Review the instructions carefully.

    • 3). See a notary public to have your proof of identity form certified. Most counties require some type of form or affidavit to prove the person requesting the birth certificate is indeed the parent or authorized party. If you were given an unofficial courtesy or souvenir copy of the birth certificate at the hospital, you may wish to take this with you.

    • 4). Send your completed application and notarized identity documents to the country recorder by mail with the proper fee--if any--enclosed. Most recorders will send a certified copy of the birth certificate to you within a few weeks.

    • 5). Visit your county recorder's office to submit the application and identity form in person if you don't want to use the mail. Country registrars usually accept applications in-person.
