How to File a W7 With My Taxes
- 1). Fill in Form W-7 with your personal information and reason for applying for an ITIN. Sign and date the form after you finish filling in all required information.
- 2). Make photocopies of your supporting documentation. If you choose to submit a valid U.S. passport, this is the only supporting document you will need to provide. If you do not submit a passport, you must provide at least two other documents, such as medical records, a birth certificate or a driver's license.
- 3). Take the photocopies of your documents, along with the originals, to a notary public to have them verified and stamped with an official seal. If you are unable to have your photocopies verified by a notary, you will need to submit the original documents with Form W-7.
- 4). Attach Form W-7 and your supporting documents to your federal tax return and mail it to the IRS. Keep a copy of your application along with a copy of your original tax return in your own records.
